Have your say on local health services

Lancashire LINK are looking for the views of people who have used NHS services since 1st April 2008.

This is your chance to feed into an annual report on NHS services in Lancashire. The views go to the Healthcare Commission – they are looking for both positive and negative experiences.

From the LINK letter:

The NHS and the County Council in Lancashire together spend very large sums of money on health and social care services every year – these are big, important services which affect almost everyone’s lives. So, it is important that we all have a say in how they make their decisions.

You can download a Word version of the survey and email it to Lancashire LINK or you can download a PDF version of the survey and send it back via FREEPOST RRXE-YYEU-ZZRB, Lancashire LINK Support Team, Interlink House, Farington Business Park, Leyland, PR25 3GG.
